Updates to the Patient Information form Copied

We’re updating the Patient Information form to improve the experience for patients and ensure that all necessary information is captured accurately.

What’s changing?

In the Advance Care section, the question, “Have you executed a Medical Power of Attorney, Directive to Physicians (Living Will), or Out-Of-Hospital Do Not Resuscitate form?” will continue to require a yes or no response.

If the patient selects “No,” they can proceed to the next question without additional input.

If the patient selects “Yes,” they will be prompted to provide additional information:

  • The name and contact phone number of their surrogate decision maker.
  • If the patient does not wish to provide this information, they must check the box labeled “I am unwilling or unable to name a surrogate decision maker.”
  • Patients with a Do Not Resuscitate can enter “N/A” in the name and phone number fields to complete the form.

NOTE: If the patient attempts to skip this section after selecting “Yes,” an error message will appear at the top of the form, guiding them on how to complete the section correctly.

The responses to both the initial question and any follow-up questions will be reflected in the PDF form sent back to the Ontada Health Forms queue and will display in the patient’s chart as a document.
