Updated Distress Thermometer form Copied

A new version of the Distress Thermometer form will be available to send to patients after this release.

The updated form includes a revised Practical Concerns section with a new selectable option: Safety.

This enhancement allows patients to more accurately communicate concerns related to their personal safety, helping care teams better assess and respond to patient needs.
