Preventative Care form updates to capture more details Copied

Now when completing the Preventative Care form, if a patient indicates they had a mammogram, they will be presented with two follow-up questions to provide more details regarding the screening (callouts 1 and 2).

If a patient indicates they received the COVID-19 vaccine, they will be required to indicate the manufacturer of each vaccine. They may choose from Janssen/Johnson & Johnson, Pfizer/BioNTech, or Moderna (callout 3).

Once the patient completes the information, these additional details will also appear in the form in your Forms queue.
