NCCN Distress Thermometer and PHQ updates Copied

Before patients begin the NCCN Distress Thermometer or PHQ-2 and PHQ-9 forms, the following statement will now be displayed:

“If you are facing mental health struggles, emotional distress, alcohol, or drug use concerns, or need someone to talk to before you visit our clinic, please call 988. This lifeline is available for a free and confidential conversation with a caring counselor any time of day or night.”

Patients can click Back To Forms List to leave the form or Continue to proceed with completing it.
