Forms updated to include logo and MRN Copied

After this release, practice forms will be updated to include the following:

Logo: Your practice may add a logo image to the top of the form. If you elect not to use a logo, a text version of your practice name will appear on the top of the form instead (callout 1).

MRN: The patient’s MRN will be added to the form alongside their other demographic information (callout 2).

NOTE: Previous forms will not be updated to include these enhancements.
