Enhanced Social Habits Form Copied

We updated the Social Habits form to ensure a more comprehensive patient health assessment. These enhancements can help improve engagement and contribute to a more personalized healthcare delivery experience.

Here’s what to expect from this update

1. Clear Yes or No Responses: Patients are now required to provide a yes or no response to four questions:
   • Do you currently drink alcohol or have you in the past?
   • Do you currently use tobacco products or have you in the past?
   • Do you currently drink caffeine or have you in the past?
   • Do you currently use recreational drugs or have you in the past?
2. Improved Data Entry: If a patient answers yes to these questions, they will be prompted to provide additional information. The fields for additional information have been updated as follows:
   • The field for choosing the type of substance features updated placeholder text, guiding patients to select an appropriate option (callout 1).
   • The field for entering the quantity of substance used only accepts positive numbers, ensuring accurate data entry (callout 2).
   • A new field has been introduced to capture how often patients use substances, offering a dropdown selection for convenience (callout 3).
3. Ensuring Data Submission: In cases where patients select yes but fail to provide additional information, the screen prompts them to enter the necessary details (callout 4). The Next button is disabled (callout 5) until all required fields are completed, preventing incomplete submissions, and ensuring data integrity.
