Depression Screening: PHQ2 and PHQ9 “Unable to assess” checkbox now requires a specific reason Copied

What’s changing

In Patient Hx > Depression Screening & Planning, the PHQ2 and PHQ9 checkboxes previously labeled “Unable to assess patient” now display as “Unable to assess for medical reasons.”

Checking the box also requires choosing one specific reason from a list of options, such as emergent situation, patient’s functional status precludes, bipolar disorder, or active cognitive impairment before the screening can be saved.

Once saved, the patient’s Depression Status under Clinical Profile > Observations updates to reflect that they were not screened along with the selected reason.

Why this matters

The updated label and required reason more clearly and accurately capture why a patient wasn’t screened, giving providers better context in the patient’s record and improving the accuracy of depression status documentation.
