Past Medical History Form Preparation Instructions Copied

What’s changing

To help patients prepare before completing the Past Medical History form, a new instructional page has been added at the beginning of the workflow outlining the information they may need to have available, including:

• Diagnoses and medical conditions
• Pacemakers and metallic implants
• Surgeries, biopsies, and other procedures
• Imaging history
• Radiation therapy history
• Family medical history

This instructional page will be displayed when patients begin both new and existing Past Medical History forms going forward.

The page is intended solely as guidance before form completion and will not be included in any downloaded or printed PDF versions of the form.

Why this matters

Completing a comprehensive medical history form often requires information that patients may not have readily available. By providing clear preparation instructions upfront, patients can gather the necessary details before they begin, helping to create a smoother form completion experience, reduce interruptions during the process, and decrease the likelihood of incomplete or abandoned forms.
