Notifications Clicked Column Added to Form Completion Report Copied

The Patient Information and Insurance Information forms and the associated PDFs will be updated so that patients and caregivers who do not have a primary care physician (PCP) can answer “No” without being prompted to enter physician details.

What’s changing

The “Do you have a primary care physician?” question on the forms will accept a “No” response without requiring physician information to be entered.

Why this matters

Previously, the forms required physician details regardless of the patient’s answer, leaving those without a PCP no valid path forward. This update removes that barrier and keeps patient data accurate.
