What’s changing

Before starting the Medications and Allergies form, patients will see a page letting them know they’ll need their list of medications, supplements/vitamins, herbs, and OTCs (including all dosing info), their list of allergies and reactions, and their preferred pharmacy information (address, phone number, and fax) to complete it.

This page appears for both new and existing forms going forward but is not included in the downloaded/printed PDF.

Why this matters

This helps patients prepare before they start filling out the form, reducing the chance they’ll need to stop mid-way to gather medication, allergy, or pharmacy details. This should lead to a smoother completion experience and fewer abandoned or incomplete forms.
