What’s changing

Before starting the Insurance Information form, patients will see a page letting them know they need their primary insurance card, secondary insurance card, supplemental insurance card, and/or prescription drug card to complete it.

This page appears for both new and existing forms going forward but is not included in the downloaded/printed PDF.

Why this matters

This helps patients prepare before they start filling out the form, reducing the chance they’ll need to stop mid-way to track down insurance or prescription card information. This should lead to a smoother completion experience and fewer abandoned or incomplete forms.
