Enhancements to EOM documentation in the VBC Care Plan Copied

In the May 2023 release, we introduced a new EOM section in the VBC Care Plan. Now, we’ve enhanced the ability to document EOM episodes in preparation for the new program set to begin in July 2023.

Added EOM billing code

The Centers for Medicare & Medicaid Services (CMS) confirmed there will only be one billing code needed when documenting EOM episodes. We update the EOM PBPM Fee section of the plan with the code so that it can be added to each episode (callout 1).

Added pagination for easier navigation

We added pagination in the Billing History section of the EOM screen to make it easier to view once several EOM charges are documented (callout 2).

View NCCN Distress Thermometer results

The Screening and Social Work Referral section now displays the information captured in the NCCN Distress Thermometer screening from the Patient Hx tab.

The new NCCN Distress Thermometer Screening area (callout 3) will carry over the screening date, distress score, plan, and any added comments for both existing and newly created VBC Care Plans.

If the patient has not completed a screening, the NCCN Distress Thermometer Screening area will display no record found under each field.
