Charge Capture Report: ICD-10 diagnosis codes sequenced by primary designation Copied
What’s changing
iKnowMed previously displayed ICD-10 diagnosis codes on the Charge Capture Report and charge interface in alphanumeric order, regardless of which diagnosis was designated as primary.
After this release, ICD-10 codes will be sequenced based on primary diagnosis designation, in compliance with payer guidelines.
The updated sequencing logic works as follows:
- Single primary diagnosis: The primary diagnosis is listed first followed by all remaining codes in alphanumeric order
- Multiple primary diagnoses: All primary codes are listed first in alphanumeric order, followed by all non-primary codes in alphanumeric order
- No primary designation: Codes are displayed in alphanumeric order (no change from existing behavior)
This logic applies to the following charge types for individual medication orders:
- E/M codes
- Procedure codes
- Drug & Administration codes
Note:
- ICD-10 sequencing for regimen medication orders remains the same.
- Care Plan charges are not affected, as only one diagnosis code can be associated with a Care Plan.
Why this matters
When ICD-10 codes were sorted alphabetically without regard to primary designation, conditions could appear first on a claim even when another condition was the true primary diagnosis. This could lead to claim denials and incorrect reimbursement. This update helps ensure claims are submitted in compliant order, reducing denials, rework, and revenue risk.
