Care Plan: Updated medical reason verbiage for acute health crisis Copied

What’s changing

The Medical Reason option available under Current Medication in the Clinical Quality Exceptions care plan has been updated.

The checkbox previously labeled “Patient is in an urgent or emergent medical situation” has been renamed to:

• Patient is in an acute health crisis

To document this exception, go to Clinical Profile > Care Plans, open or add a Clinical Quality Exceptions care plan, expand Current Medication > Medical Reasons, check the box next to Patient is in an acute health crisis, and save your changes.

Why this matters

This updated label ensures that the exception documented in iKnowMed maps precisely to the MIPS-130 requirement, supporting accurate quality reporting for practices participating in CMS quality payment programs.
