Use Patient’s Current Weight, BSA, and/or Height for Formula Dose Based Orders Copied

A new option has been added to formula dose-based orders that allows users to update the order based on the patient’s current weight, BSA, or age, even if the weight change was below the threshold that would trigger a weight change warning.

The calculator will display the weight, BSA, and age (when relevant) that was used in the prior order, as well as the most recent values (see callouts 1 and 2), allowing the user to decide which values to use.

For Carboplatin orders, the calculator will also display the option to use Cockcroft-Gault Ideal Body Weight-Miller Formula for Creatinine Clearance (see callout 3).
